Hiten is a 1984 graduate of Northwestern University/Feinberg School of Medicine Programs in Physical Therapy. He started his private practice in Pleasanton, CA in 1985 and an additional clinic in Walnut Creek, CA in 2004. He has taken numerous continuing education courses in learning to treat difficult/chronic musculo-skeletal problems. Other areas of specialization includes treating feet, spine, headaches including tension and migraine and TMJ. Mr. Dave’ has genuine care for his patients and is evident from the outcomes his patients achieve at the end of their visit. Mr. Dave’ has been a member of the American Physical Therapy Association including the California Chapter since 1985 and is an APTA Credentialed Clinical Instructor.
Outside his working hours, Hiten enjoys reading, practicing meditation and spending time with his wife and two daughters.
Falguni “Kim” Davé, PTA With a Bachelor’s degree in Microbiology and Chemistry from India, Kim always had a passion to help and work with people. She realized her aspirations in physical therapy work which led her to attend the Physical Therapy Assistant program at De Anza College in Cupertino, CA. She has been a practicing Physical Therapy Assistant since 1990 and continuing to take additional courses to enhance her career path. Kim enjoys working with the elderly population especially with balance disorders.

Hi, I’m Alexa! I received my bachelors degree in Biological Sciences at UC Santa Barbara and my Doctorate in Physical Therapy at Western University of Health Sciences of Pomona. I have experience in working in outpatient and inpatient settings with varying demographics ranging from weekend warriors to geriatrics. I especially enjoy helping patients reach their goals of returning to their active lifestyle, whatever that may be. I see value in the patient-provider relationship and understand that getting to know the patient is key in tailoring a treatment plan that works for them. I believe in treating based on the biopsychosocial model and assessing the patient as a whole, not just their diagnosis! In my free time I love hiking, weightlifting, playing tennis and cooking.
Hey everyone! My name is Nadia Alazzeh and I am a licensed Doctor of Physical Therapy here at Back on Track. Before getting my doctorate, I received my undergraduate degree in public health and psychological studies with a great focus on outreach to underserved communities. What I love the most about physical therapy is the holistic focus of healing through movement, muscle strengthening/lengthening, positive mindset, and hands-on mobilizations. I am looking forward to meeting with you to work together and get you back to what you love doing!

In middle school, my brother broke his leg while we were messing around in the back yard. My mom said that since I was partially to blame for his injury, then I was to be his main caretaker at home. Naturally, it started with “get me this and that, get me ice cream, etc.”, but eventually, I was helping him with his crutches and then his leg and balance exercises. I felt a sense of pride with my brother’s full recovery and decided to pursue P.T. professionally. After earning a B.S. in Biology, I studied at Boston University where I earned my MSPT in Physical Therapy.
I worked at and eventually managed an outpatient P.T. clinic in Vallejo, CA for ten years, then spent 7 plus years at a post-acute rehab center in Danville, CA where I gained experience in treating patients with strokes, amputations, total hip and knee replacements, fractures from falls and a host of other diagnoses.
Outside of work, I fancy myself as an amateur entomologist and have an extensive butterfly collection. As for my brother – he owns an ice cream shop in L.A.
Hi, I’m Angela! I was born and raised in Stockton, California. I received my Bachelor of Science in Health and Exercise Science at the University of the Pacific in 2017. I then continued my education at the University of the Pacific and obtained my Doctorate in Physical Therapy in 2022. I was able to participate in a variety of outpatient orthopedic clinical rotations in San Joaquin and Solano County. I enjoy treating my patients in a holistic and individualized approach where I can work hands-on to help them achieve their functional goals. Outside of the clinic I enjoy spending time with my family and trying new restaurants/ breweries. I look forward to working with you!
I am a Bay Area native and graduated from UC Davis with a degree in Exercise Biology. Then, I earned my Doctorate in Physical Therapy from the University of the Pacific. I have experience in a broad range of treatments and modalities, working in private outpatient orthopedic clinics. Some of my favorite aspects of being a physical therapist are the one-on-one interaction with patients and guiding them as they progress through their treatment. Outside of work, I enjoy spending time with my family and friends, going to concerts and sporting events, hiking, and cooking.
Hello there! My name is Vincent, and I am a PTA born and raised in the Bay Area. Originally a business major from SJSU, my PTA journey began following my graduation from Carrington College. Very few get to experience a second chance at life, which is why I truly believe that with all life’s curveballs, you can accomplish great things with a positive attitude.
Outside of work I love sports, fitness, all things comic book related. But most of all, I love spending time with the love of my life—my dog, Stanlee. My wife and I enjoy exploring with him and binge watching shows together.

Hey! I’m Ishani. I received my Bachelor’s in Physical Therapy from India and later received my DPT from Loma Linda University in California. My training and clinical experience has included working in several different settings such as outpatient orthopedics, in-patient acute and post-acute care, neurological rehabilitation and cardio-pulmonary rehabilitation. After working in these various settings I have found outpatient orthopedics to be an area of my prime interest.
I firmly believe that along with providing exceptional skilled physical therapy, values such as kindness, patience and holding unconditional positive regard toward patients is necessary for patient care. I strive to empower my patients/caregivers with lucid translation of my medical knowledge. It is important for me that my patients are able to understand their pain and purpose of physical therapy and therefore have complete control of their care. I try to approach my patients holistically and emphasize the importance of building small habits that will put them in the right direction toward their health. I have extensive experience in dealing with orthopedic post-operative conditions, overuse/repetitive strain injuries, chronic pain, postural dysfunctions, incorrect gait, work-related injuries and traumatic injuries. I am fluent in English, Hindi, Gujarati and Marathi. In my personal time I enjoy travelling, taking my dog on hikes, meditating, practicing yoga and painting.
I earned doctorate degree from Arcadia University of Pennsylvania. I chose to join this team because of the commitment to quality of care that this team provides to their patients daily. I enjoy my work with a variety of patients and helping them achieve their goals. I have extensive experience in dealing with orthopedic post-op conditions, repetitive strain injuries, fractures, stroke, postural dysfunction, and balance disorders conditions.
When I treat my patients, I always make sure they feel comfortable and able to express their feeling through questions or concerns about their condition they may have. I believe, listening their concerns and answering their questions plus educate them to understand their symptoms and condition are very important in the healing process.
My main goal is to see my patients through their rehabilitation with support, treatment, patience and build relationships that can lead to a better health success.
I also have a strong interest in pursuing further specialize courses to expand my knowledge in this everyday changing world to keep up to date with latest treatments and techniques.

Dr. Apoorva born and raised in India, achieved a Bachelor’s degree in Physical Therapy from DES college of physiotherapy and Master’s degree in Orthopedic rehabilitation from Sancheti hospital, Pune. While studying she undertook research, published research papers, developed manual skills and enhanced her clinical reasoning and decision making ability. She expanded her knowledge through specialization in Musculoskeletal Rehabilitation and refined her skills with clinical experience while working within an inter-disciplinary team of orthopedics, neurologists, occupation therapists and orthotists, she treated a wide array of patient population including acute and chronic conditions, postural syndromes, non- surgical as well as post-operative orthopedic cases.
Apoorva added another feather to her hat with a Clinical Doctorate in Physical Therapy from Loma Linda University, United States, which widened her horizon to holistic care. She continues to learn new techniques and keep herself updated with the current evidence. With proficient communication skills and by utilizing the tools such as Maitland mobilization, Myofascial/soft tissue release, Kinesio-taping, therapeutic exercises, ergonomics and more, she empowers her patients to get better.
She finds herself passionate to be in patient’s shoes and walk through the process with them. With eagerness to help others and by incorporating empathy and patient-centered care, she strives to get her patients back to their functional self.
When not working she enjoys spending time with family and friends, hiking, cooking and listening to music.
I’m Dr. Shilpa, a proud alumna of Rajiv Gandhi University in India. My academic journey peaked in 2021 when I earned a Doctorate in Physical Therapy from the University of Montana. I embarked on my physical therapy career in 2008 at an Obgyn clinic, later transitioning to a multi-specialty hospital where I teamed up with a plethora of specialists. My forte includes treating acute and chronic pain, orthopedic post-operative conditions, repetitive strain injuries, and more. A defining moment in my career was collaborating with elite sports teams, such as the India National Women’s Boxing Team, U-16 Women’s Basketball, and the Tennis Federation. Moreover, in 2015, I was privileged to work closely with Olympian Mary Kom. My commitment to patient care is unwavering. I continuously hone my skills in therapeutic techniques, including Maitland mobilization, Myofascial release, and Kinesio-taping, ensuring I offer the pinnacle of care. I advocate for a comprehensive approach to care, emphasizing the crucial role of educating patients. Away from the clinic, I savor serene moments immersed in nature, with camping and swimming being my personal retreats. Additionally, I take pleasure in preparing delicious meals for my two cherished children, aged 4 and 1.5.
Ketaki is a Physical Therapist who graduated from DES college of Physiotherapy in India. Ketaki moved to the Bay area after getting married in 2014 and has been living here since then. Her special interest is in pre and post surgical orthopedic conditions. She also has extensive experience in treating other orthopedic conditions like muscle and ligament injuries, postural issues, degenerative conditions and more. She strives to skillfully and compassionately guide her patients to their goals of returning to normal functional activities after major physical trauma. She is fluent in speaking English, Marathi and Hindi. In her free time she enjoys listening to music and watching movies.

Hi, I’m Ada and have lived in the bay area for most of my life. I am a UC Davis alumni with a BS in Biology that is passionate about the healthcare industry. I decided to pursue a career in physical therapy after rehabbing my own sports related injuries and realizing how life changing it can be. I graduated with my Doctorate in Physical Therapy from Samuel Merritt University where I gained experience in various settings including inpatient rehab, outpatient orthopedics/neuro, and pediatrics. Since then I have worked with a diverse group of patients with orthopedic, vestibular, and neurological conditions.
In my spare time I enjoy reading, baking, playing tennis or badminton, and spending time outdoors with my dogs.

Hi! I am Prajakta, but you can call me Praj. I am a physical therapist specializing in orthopedic and sports injuries. I earned my bachelor’s degree in physical therapy from Maharashtra University of Health Sciences, Mumbai India in 2012. I then pursued a master’s degree from University of South Australia specializing in Musculoskeletal and Sports Injuries in 2014. Over the years, I have worked in different cities around the world. It has helped me gain unique experiences not just professionally but personally to help understand my clients better no matter what their background may be.
I specialize in a variety of manual therapy techniques and have been trained in joint mobilization techniques, taping techniques, soft tissue release techniques and Pilates for rehab. I enjoy a methodical and inquisitive approach while dealing with patients’ issues. I like treating conditions with a holistic approach and prefer working with professionals from varied specialties to help assist my patients achieve their goals.

Hey, I’m Michael! I’ve lived in Livermore most of my life, but I’ve spent time in Rohnert Park where I earned my BS in Kinesiology at Sonoma State University, as well as Stockton where I earned my Doctorate in Physical Therapy at the University of the Pacific. I have a number of experiences in different outpatient clinics, sports clinics, as well as inpatient hospital work. I am a firm believer in physical therapy as I too have gone through many rehab periods in my own life. In my free time I enjoy many outdoor activities such as fishing, basketball, and golf.
Hi, I’m Anita! I grew up in Fremont and earned my Bachelor’s of Science degree in Human Biology from the University of California, San Diego and my clinical doctorate in physical therapy from Nova Southeastern University in Fort Lauderdale, FL. I completed my clinical education throughout the country from the Mayo Clinic in Rochester, MN to Spine and Sport Physical Therapy in San Marcos, CA where I was fortunate enough to work with a range of patients, including both pediatric and geriatric patients. I am very passionate about helping my patients achieve their optimal health through movement and helping them return to the activities they enjoy doing the most. When I’m not at work, I enjoy spending time with my husband, our daughter, and our Goldendoodle Aero.

Hello, I am Megan Darling. I have been a licensed Physical Therapist Assistant since 2022. However, I have worked in Physical Therapy for 10 years starting as a Physical Therapist Aide, while I studied for my Bachelor’s in Health and Exercise Sciences at University of the Pacific. After receiving my bachelors degree I worked as an Athletic Director and Personal Trainer for 2 years. I then returned to school at Carrington College to receive my degree and license as a Physical Therapist Assistance. Outside of work I am a wife and mother to three young children who mean the world to me! As a family we enjoy trips to Disneyland as well as being outdoors. Can’t wait to meet you!
